Revised submission files, reviewer feedback and revision history

Fixed revised uploads showing outdated files, improved removal of unfinished reviewer assignments, and made referee feedback readable with formatting and links. Editors can now see each revision request’s correction notes in the submission timeline, including after resubmission.
